Inflammatory bowel diseases (IBD) with a very early onset (VEO) in a manifestation have an atypical or worn out clinical picture. With the development of the disease, IBD with a VEO acquires a more aggressive phenotype than IBD in older children. The article contains description of the patient (5 y.o.) who arrived at the clinic with complaints of frequent acute respiratory diseases of a protracted nature, accompanied by episodes of liquid stool, which was caused by the manifestation of Crohn’s disease with a very early onset.
An important point of differential diagnosis was the determination of the level of fecal calprotectin and specific markers of IBD. The presented clinical case demonstrates that patients with very early onset of IBD need more intensive immunosuppressive therapy. The need for the application of recombinant monoclonal antibodies in the treatment of patients with very early onset of IBD with a VEO is discussed.
