Risk factors for recurrence of intraocular pressure increase after planned cataract surgery on glaucomatous eye with target intraocular pressure

During 3 years, dynamic observation of 62 eyes after phacoemilsification (PE) with POAG was performed. Recurrence of IOP increase in 22 eyes were revealed — the 1st group of patients. The 2nd group consisted of 40 eyes in which IOP level remained at initial level. The frequency of factors of PE complication in the 1st group was higher (rigid pupil, denser nucleus, existence of incomplete dislocated lens). In the 1st group were applied iris retractors and the influence of ultrasonic energy was longer and more intense. The initial existence of changes, morphologically adverse for PE implementation, significantly raised intraoperative traumatization of eye structures. It can promote disorders of eye hydrodynamics balance, leading to recurrence of permanent IOP increase. Considering the obtained data, PE in eyes with POAG with target IOP requires maximally nontraumatic surgical approaches.
