Objective. The study was conducted for a comparative analysis of the results of surgical treatment in degenerative lesions of the lumbar spine using dynamic interspinal stabilization and without the use of an implant. Material and methods. Patients were selected with the most similar pathology of the spine. Selection of patients was made taking into account the neurological examination; assessment scales, instrumental methods of research were used.

Results. Dynamic stabilization has demonstrated its positive effect in the form of an improvement in the condition of the patients, better functional and anatomical preservation of the vertebral motor segment in the long-term period.

Conclusion. Interswitch stabilization has shown its high efficiency, especially in the remote periods after the operation and allows significantly slowing down the decrease in the height of the intervertebral space.

Key words: spinal surgery, spine, implant, canal stenosis, indications for surgery.
