     To assess the frequency of the prevalence of pneumococcal carriage in preschool children; to determine the serotype composition and to study the antibiotic sensitivity of pneumococci.

    Material and methods. Phenotypic identification of S. pneumoniae was performed on the basis of morphological, cultural data. Species identification of cultures was confirmed by MALDI-TOF mass spectrometry. Testing of antibiotic resistance and interpretation of the results were carried out

    according to the Clinical Recommendations «Determination of the sensitivity of microorganisms to antimicrobial agents» (version 2015), EUCAST (2015). Molecular serotyping of S. pneumoniae strains was performed by multiplex PCR (MPCR).

    Results. 484 children aged 2–7 years, attending preschool institutions of the Republic of Tatarstan, were studied. The S. pneumoniae carrier status was 25.9% of children living in the city and 37.5% of children from rural areas with the associative nature of the nasopharyngeal microbiota in children with a long carrier period. Analysis of the serotype composition of pneumococci showed the dominance of «vaccine» serotypes; the appearance of «non-vaccine» serotypes; tendency to increase in the number of encapsulated pneumococci. An insignificant increase in the antibiotic resistance and incidence of polyresistant nasopharynegeal pneumococci was revealed.

    The conclusion. The study of pneumococcal nasopharyngeal carriage with potential virulent properties and antibiotic resistance is a prerequisite for the development of preventive measures and antimicrobial therapy algorithms.
