The article presents the features of treating consequences of severe lesions of ankle bone in 36 patients (the total of 40 lesions), hospitalized in the Department of adult traumatology of VTO Scientific-research Center — Science and Practice Center for Trauma of Republic Clinical Hospital in 1990-2022. Among the most severe complications of ankle bone is the development of avascular necrosis of ankle bone with fragmentation and lysis of fragments. The development of avascular necrosis of ankle bone took place in most cases of lesions and in 28 cases of fractures. The most severe complications took place in fractures of femoral neck and block with violations of congruency in adjacent joints and in full fracture-dislocations. The most feasible method of treating severe avascular necrosis of ankle bone with fragmentation and lysis of fragments was necrotomy with the formation of tibiotarsal arthrodesis.
