On the effectiveness of using the domestic pupillary dilators in high-tech outpatient surgery of complicated cataracts

The article evaluates the efficacy and safety of the use of domestic dilatation pupil in the outpatient surgery of complicated cataract in patients with narrow rigid pupils. 27 patients with complicated cataracts and narrow rigid pupil underwent outpatient phacoemulsification with IOL implantation. Two mechanical domestic dilators were used to dilate a pupil ― «Malyugin ring» produced by «Eye Microsurgery» and «Ice» JSC. The average age of patients was 63.8±7.8 years; 21 females (78%) and 6 males (22%). Average BCVA was 0.12±0,11; IOP was 19.9±3.8. There were no operative complications. In the postoperative period 5 (18%) patients had ocular hypertension, which was offset by a medication; 4 (15%) patients had corneal edema, which reversed itself in a week after the surgery. As a result of the treatment, uncorrected visual acuity was 0.52±0.02. The average spherical equivalent of clinical refraction was 0.41±0.38 diopters. Target refraction was achieved in 91% of patients. In 3 patients with «AcrySof IQ Toric» implants astigmatism decreased to 0.53±0.04 diopters. The loss of cells of posterior corneal epithelium was 4.9±1.9%. Both models of mechanical dilators of pupil produced by «Eye Microsurgery» and «Ice» JSC provide sufficient mydriasis strictly in the area of the pupil, they are non-traumatic, retain a stable position throughout the operation, are inserted and removed through a primary tunnel incision, ensure the preservation of diaphragmatic function of the pupil. In our practice of outpatient surgery of complicated cataracts we more often (77.7%) used the «Malyugin ring» produced by «Eye Microsurgery», which provided a slightly larger pupil diameter due to the peculiarities of its design.
