Noncompact myocardium is a form of cardiomyopathy, which characteristic feature is a pathological hyper-trabecular of the left ventricle myocardium, with a normal and noncompact myocardium layers. The pathogenesis and clinical manifestation are composed of the leading syndromes: heart failure, arrhythmias, and thromboembolic syndrome. Taking into account the non-specific character of these manifestations, unfavorable prognosis and high lethality, the instrumental methods play the major role in diagnosing this disease. The article describes approaches to multi-modal radial semiotics and differential diagnostics of noncompact myocardium. The diagnostic algorithm includes the role of single-photon emission computer tomography (SPECT) of myocardium with Tc99m «Technetril», magnetic-resonance tomography (MRT) of heart with contrasting paramagnetic medications based on gadolinium.

Key words: cardiomyopathy, noncompact myocardium, single-photon emission computer tomography (SPECT) of myocardium, magnetic resonance tomography (MRI) of heart.
