    The purpose — to characterize myofascial pain syndromes, their pathogenetic mechanisms, and on this basis to give diagnostic guidelines, principles of differentiated treatment for primary care physicians.

    Material and methods. The article presents the literature data and, to a greater extent, the results of personal studies related to propaedeutics, diagnosis and treatment of myofascial pain syndromes.

    Results. Diagnosis of myofascial pain syndromes remains extremely difficult, especially for the primary care physician. Based on the analysis of world literature and personal observations over the past 25 years, the main clinical manifestations of myofascial pain syndrome in the algic and trigger stages are presented. The latter constitute the main clinical picture of many myogenic syndromes of various etiologies. The technique of differential diagnosis is described, which allows to distinguish the algic stage from the trigger stage and to avoid overdiagnosis. Given the pathogenesis and clinical picture of myofascial pain syndromes, differentiated treatment is proposed.

    Conclusion. The presented clinical picture of myofascial pain syndromes, taking into account the differences between the logical stage and the trigger one, will be useful in prescribing pathogenetic treatment not only for primary care physicians, but also for neurologists, manual therapists, osteopaths, rheumatologists and rehabilitologists.
