Influence of hypoxemia in acute period of ischemic stroke of the development of post-stroke cognitive impairments

The purpose — to study the effects of hypoxemia of the most acute period of the first developed mild and moderate ischemic stroke (IS) on the formation of cognitive impairment (CI) in the early recovery period of stroke.
Material and methods. 46 patients were examined (23 women, 23 men). There pre-stroke cognitive status was assessed using the IQCODE scale. The cognitive functions in the debut and further were investigated using the Mini-Mental State Examination (MMSE), the Montreal Cognitive Assessment Scale (MOCA) and the P300 cognitive evoked potentials. Obstructive sleep apnea syndrome (OSAS) was diagnosed using a non-invasive technique of pulse oximetry, which is a recognized screening method for evaluating of that condition.
Results. Pre-stroke CIs were diagnosed in 24% of patients in the main group; post-stroke CIs developed in 96%. The majority of patients in the acute period of a stroke with a background of OSA (98%) had moderate and severe CIs which partially regressed later on. In general, there was a trend of CI growing by the time of the completion of the IS early recovery period.
Conclusion. The data obtained indicate a high prevalence of OSA in patients in the acute period of IS, significantly affecting the aggravation of CIs existing before the stroke and the formation of recurrent ones, mostly moderate, as early as in the acute and early post-stroke periods.
