Hip arthroplasty in patients with ankylosing spondylitis

Lesion of the hip joint (coxitis) in patients with ankylosing spondylitis (АS) is an adverse prognostic sign and can lead to endoprosthesis (EP) of the joint.
Objective ― to evaluate the results of hip joints replacement in patients with АS under the dynamic supervision of a rheumatologist and an orthopedist within the first year after the operation.
Material and methods. The results of hip EP were analyzed in 22 patients with AS. 16 of them were positive for HLA B27, with high activity (ASDAS) ― 58.3% of them, with moderate activity ― 41.7%. Dynamic observation by a rheumatologist and an orthopedist was carried out before the operation and 6 and 12 months after it.
Results. The effectiveness of large joints arthroplasty with a significant reduction of pain (VAS) in the first month after EP, improvement of functional ability (BASFI), and reduction of AC activity (ASDAS) in 6 and 12 months after EP was demonstrated. There were no complications after the surgery. A clinical example of a patient with severe ankylosing spondylitis and the effectiveness of arthroplasty is presented.
Conclusion. EP of hip joints in patients with AS is effective not only in terms of improving the functional ability and pain relief, but also reducing the disease activity in general.
