  • Efficacy of platelet-rich plasma for treating patients with knee osteoarthritis

    The study objective was to determine the therapeutic efficacy of platelet-rich plasma in the treatment of patients with stage II and III knee osteoarthritis (OA).

    Material and methods. 86 patients with OA of the knee joint of II and III stages by J. Kellgren and J. Lawrence classification were divided into 4 clinical groups. Groups 1 and 3 (24 and 20 patients) included patients with OA of stages II and III, who received three intra-articular injections of PRP, 1 injection per week. Patients with stages II and III of OA in groups 2 and 4 (20 and 22 patients) received intra-articular injections of hyaluronic acid with the same rate. The results were evaluated by SVS, 10-point VAS and Lequesne’s scale at 1, 3, 6, and 9 months after the therapy.

    Results. Assessment by SVS after 1 month showed a positive result in 17 (70,8%) and 10 (50%) patients of groups 1 and 3, respectively. In the groups using hyaluronic acid (2 and 4) after 1 month, 12 (60%) and 9 (40,9%) respondents noted a positive effect. After 9 months, according to SVS, 9 (37,5%) and 5 (26,4%) patients admitted a positive result in groups 1 and 3. In groups 2 and 4, the positive effect was preserved in 4 (22,2%) and 4 (19,1%) patients, respectively.

    The VAS score showed the maximum decrease in indicators after 1 month. After 9 months from the beginning of the study, patients of all groups noted negative dynamics, which was more pronounced in groups with stage III osteoarthritis regardless of the treatment method.

    The Lequesne scale in patients from 1 to 4 clinical groups 1 month after the course of treatment indicated an improvement in the quality of life of patients. After 9 months with the return of pain, we noted an increase in the severity of the disease.

    Conclusion. Intra-articular injection of PRP to patients with stage II and III of knee OA makes it possible to reduce the intensity of the pain syndrome to a greater extent and promotes better restoration of the knee joint function during 9 months of observation as compared with the use of a hyaluronic acid preparation.
