The results of surgical treatment of patients with retinal detachment, complicated by anterior proliferative vitreoretinopathy, are analyzed in the article. Patients of the 1^st group underwent vitreoretinal surgery with retinotomy, patients of the 2^nd group underwent vitreoretinal surgery with retinotomy and scleral buckle. The results of preliminary studies indicate that the use of scleral buckle as an additional element of vitreoretinal surgery optimizes the sanation of the basal vitreous, weakens circular tractions and compensates for the forward displacement of the peripheral retina.
