Difficulties of management of a pregnant woman with familial hypertriglyceridemia: a clinical case

The article describes the clinical case of a pregnant patient with extremely high levels of triglycerides in blood, which in history had three episodes of pancreatic necrosis, one of which developed during the first pregnancy. The state of pregnancy did not allow to resort to the drugs of choice for hypertriglyceridemia. The council of doctors of different profiles decided to use the plasmapheresis. The patient received 10 sessions of plasmapheresis with an interval of 2 to 12 days. After plasmapheresis, the triglyceride levels decreased by 30% from 55.4 to 38.08 mmol/l. Positive dynamics of the level of triglycerides made it possible to avoid acute pancreatitis. The pregnancy successfully ended with birth. Currently, during the lactation period, the patient is taking omega-3 acids, it is planned to use fibrates.
