The purpose — to present the literature data concerning the features of diagnosing intravascular B-large cell lymphoma (IVLBCL), a rare form of diffuse B-cell lymphomas, more often detected at autopsy, and a clinical case.

Material and methods. A description of the clinical observation of IVLBCL is presented, verified by immunohistochemical autopsy, under the guise of dementia in a patient diagnosed with a caecum tumor.

Conclusion. The article is of interest to practitioners, as IVLBCL is often diagnosed only at autopsy, masked by other various diseases during life. The key to IVLBCL diagnosis is a skin biopsy and immunohistochemical study.

Key words: intravascular large B-cell lymphoma, dementia, intestinal tumor, immunohistochemical analysis.
