Diastolic myocardial dysfunction in patients with different clinical and laboratory features of rheumatoid arthritis

The risk of cardiovascular disease in patients with rheumatoid arthritis (RA) is 1.5 times higher than the population average. This is determined by the inflammatory and metabolic changes that occur in RA. Tissue Doppler echocardiography is an effective non-invasive method for diagnosing the state of the myocardium.
The aim of the study was to identify the relationship between structural and geometric remodeling of heart and the clinical features of rheumatoid arthritis. The study included 86 women and 9 men with diagnosed RA. Out of 95 patients, 27 patients were found to have type 1 diastolic dysfunction as a result of tissue dopplerography: 9 patients had diastolic dysfunction of left ventricle, 3 patients ― of right ventricle, and 15 patients ― of both ventricles. In addition to tissue Doppler imaging, all patients had results of blood test for erythrocyte sedimentation rate, circulating immune complexes, levels of antibodies to the modified citrullinaemiavimentine (anti-MCV), levels of matrix metalloproteinase-9 (MMP-9), and radiography of the affected joints. RA activity was estimated by DAS-28 disease activity index. The results of the study were as follows: elevated levels of anti-MCV and MMP-9 correlate with impaired diastolic function of the right ventricle, and increased activity of the disease leads to its systolic dysfunction. In addition, the duration of RA worsens the diastolic function of both ventricles, andan increase of the duration worsens the myocardial dysfunction.
