    The article gives the results of the assessment of indicators of nonspecific protease granulocytes and their endogenous inhibitors in 93 patients with acute rhinosinusitis that has developed in the setting of an acute respiratory viral infection; 45 people with catarrhal form and 48 — with purulent. To study the activity of proteases and their inhibitors in blood serum and sinonasal secretion in patients the enzymatic methods were applied: determination of elastase, trypsin and antitrypsic activity and acid-stable inhibitors. There were studied changes in a non-specific proteinase-inhibitory system: a nonspecific response manifested by progressive growth of proteolytic activity and level of proteinase inhibitors in blood in the course of catarrhal and purulent rhinosinusitis, whereas in sinonasal lavages in case of purulent rhinosinusitis was observed a 44.1% reduction of proteinase inhibitors compared with the norm, in case of catarrhal — 67.4%. These changes reflect the nature of the inflammatory process and may indicate the development of destructive changes in the mucosa, favorable to chronic rhinosinusitis.
